Community Choice Waiver

The Community Choices Waiver (CCW) offers an alternative to nursing facility care for seniors and individuals with adult onset disabilities. Through a comprehensive assessment, recipients receive a budget based on their needs, allowing them to customize an individual service package. The CCW encompasses a diverse range of services, such as support coordination, nursing and therapy assessments, in-home monitoring systems, home modifications, assistive technologies, personal care, home-delivered meals, and respite care for caregivers.
